Optional Enhanced Service
Second, we want to share with you an optional service from Promised Legacy where you may identify specific accounts that you have details about, like:
Life Insurance
Policy numbers and insurer details
Pension Accounts
Account numbers and providers
Financial Institutions
Banks and credit card providers
On this form, you may enter a limited number of additional notifications, where we will reach out to those providers to coordinate their ability to receive a First Notice of Loss.
This service is Free to your family.
Please keep in mind: This is the only time this service option will be available for your family. You may also choose to decline.

Terms and Conditions
You will be presented with our Terms and Conditions. Your options are to accept or decline. If you decline, that will end our journey together.
Authorization & Signature
When you accept the Terms and Conditions, you will be presented with Promised Legacy's Authorization for Release of Death Certificate Information. You will be required to sign your name. That's it!
Status Report
We will email the email address you've provided with a status report of the connections we were able to make 7 to 14 days from the transmission date.
